First, call ActivStyle and one of our compassionate incontinence consultants will help determine your eligibility. The consultant will collect some basic information necessary to determine eligibility.
- Personal Information – Your name and address so we can contact you and ship your supplies.
- Medical Condition and Diagnosis – This information will help the consultant recommend the right products to meet your needs.
- Doctor’s Information – We’ll need your doctor’s name and contact information so we can secure a prescription for your supplies and complete other required paperwork.
- Insurance Information – We’ll need your current insurance plans and member/policy numbers.
Step 2: Verify Insurance
Our team will verify your insurance is valid and active and check to see if the supplies you need are covered by your plan. Then we will complete the necessary paperwork your plan requires so that we can bill your insurance.
Step 3: Secure a Doctor’s Prescription and Medical Information
